A major police operation is underway after four people, including a police officer, were injured after a crash and suspected stabbing in Sydney’s south.

A two-vehicle crash unfolded at the intersection of the Princes Highway and Old Bush Road at Engadine just after 9am on Sunday.

Police arrived to find a man, who appeared to be suffering stab wounds, allegedly running from the crash scene.

Paramedics are understood to be treating up to five people, with one person reportedly going into cardiac arrest.

Reports to Ambulance NSW note there was one patient “with multiple stab wounds”, while another has a stab wound to their abdomen, and another has a broken arm.

The police officer is understood to have been slashed on their arm, suffering 30mm lacerations to their lower wrist.

A critical patient, a 40-year-old woman, was taken from the scene to St George Hospital for multiple stab wounds.

In total, four people have been taken to hospital.

Cronulla Sharks chairman Steve Mace witnessed the crash, and he said he came face-to-face with the knife-wielding man.

Mr Mace said he ran to help the screaming passengers’ but struggled to open the car door.

“I ... finally jammed it open and all of a sudden, I see him just slashing her,” he said in a video posted by the Daily Telegraph.

“(He was) just cutting her neck, her breast ... in the sides, wherever he could go.”

According to the Chairman, another man was injured at the scene while trying to help the passenger.

The attacker is allegedly a conspiracy theorist who attempted to flee the scene.

“It was like a stand-off, which felt like an hour,” Mace said.

The Chairman was not critically injured; he only received a small cut on his hand.
